The Interagency Forms page provides official documentation required for collaborating with the Cheasequah Nation across tribal, local, state, federal, and international jurisdictions. These forms support coordination, compliance, verification, and data-sharing for recognized projects, partners, and government entities.
Instructions for Use
All forms must be completed electronically or in black ink.
Submit via encrypted email to: govrelations@cheasequahnation.org or through the secure upload portal.
Processing time: 7–14 business days (subject to review).

Interagency Collaboration Request Form
For initiating project-based partnerships with Cheasequah Nation departments.
Required for cross-agency program support, funding collaborations, or research access.
Tribal Identity Confirmation Form
Submitted by schools, employers, health organizations, or agencies requiring identity or exemption confirmation for services.
Connects directly to the Cheasequah Nation’s secure verification database.
Diplomatic Engagement Form
Used by consular staff, international representatives, and tribal diplomats seeking to engage with the Cheasequah Embassy.
Includes treaty acknowledgment and cultural protocol confirmation.
Memorandum of Understanding (MOU) Template
Standard agreement form outlining joint goals, responsibilities, and data protocols.
Used for institutional, educational, and governmental partnerships.
Law Enforcement Verification Request
Allows domestic and international enforcement bodies to verify a UCIN, UMIN, or UBIN.
For use in sovereignty compliance, status correction cases, or lawful tribal recognition.
Data Sharing Agreement (DSA) Request Form
Required when requesting non-public records or statistical data for authorized research.
Must include project scope, privacy policy, and duration of use.
